Mare Reproductive Services Contracts
2024 Embryo Transfer - this packet includes contracts for breeding, embryo transfer, and boarding.
We request a copy of your mare's registration papers, current negative Coggins, copy of the stallion's breeding contract (if breeding), and a down payment of the "base fee" for your selected service(s) when dropping off your mare(s).
Other Contracts and Forms
Boarding Contract - If your horse will be staying with us for any length of time, please fill out and sign this contract.
Consent Form - If your horse will be having any procedure done (dental, surgery, treatment, etc.), please fill out and sign this form.
New Client Form - If you are a new client to the clinic (Welcome!), please fill out and sign this form.
Additional Animals (New Client) Form - If you are a new client with more than three animals involved in your first appointment with us, please fill out this form, in addition to the new client form.
